Before Your Visit
If you are unable to keep your appointment, please contact us as soon as possible at (662) 621-1915 or 1-888-73-HEART (43278). Please be sure and bring the following items with you when you come see the doctor:
1. Copy of insurance card
2. Social Security Card
Also, please notify the receptionist of any changes to your current address, telephone, or other contact information as well as your current insurance.
